Types of leave
Intermittent, continuous, and reduced-schedule leave each mean something different.
Continuous leave
A period away from work when symptoms or treatment needs make working medically inappropriate.

Do I need an official diagnosis?
Employers may require sufficient medical certification, but a diagnosis is not necessarily required to appear on the certification. The clinical evaluation determines what information is appropriate to document.
Can House of Haven complete my employer's form?
When clinically appropriate and within scope, House of Haven may review and complete relevant medical certification forms based on the evaluation and available clinical information.
Can I come only for paperwork?
Documentation must be supported by a legitimate clinical evaluation and professional judgment. House of Haven does not provide unsupported or guaranteed paperwork.
