By accessing or using the House of Haven Mental Health website, scheduling services, or receiving care, you agree to the following Terms and Conditions. These terms are governed by the laws of the State of Kansas.
1. Services Provided
House of Haven Mental Health provides outpatient psychiatric mental health services, including but not limited to:

Psychiatric evaluations

Medication management
-
Therapy and counseling services

Telehealth and in-person appointments
All services are provided by a licensed Psychiatric Mental Health Nurse Practitioner in accordance with Kansas state laws and regulations.
2. No Emergency Services
House of Haven Mental Health does not provide emergency or crisis services.
If you are experiencing a mental health emergency:

Do not use this website or messaging system for urgent matters
3. Appointments & Scheduling

Appointments must be scheduled in advance.

A deposit may be required to secure your appointment.

Appointments may be conducted in person or via HIPAA-compliant telehealth platforms.
4. Deposits & Payments

Deposits are non-refundable.

Deposits are applied toward your appointment fee once services are rendered.

Failure to attend your appointment without proper notice will result in forfeiture of the deposit.
5. Cancellation & No-Show Policy

Cancellations must be made at least 24 hours in advance.

Possible discharge from care after repeated occurrences
6. No Refund Policy
No refunds will be issued for services that have already been provided.
By scheduling an appointment, you acknowledge and agree that:

7. Telehealth Services (Kansas)
By participating in telehealth services, you acknowledge:

You are physically located in Kansas at the time of service

Telehealth services are subject to Kansas laws and licensing requirements

Technology limitations or disruptions may occur
8. Patient Responsibility
You agree to:

Provide accurate health and personal information

Follow treatment recommendations

Communicate respectfully with staff

Pay all fees associated with services
Failure to comply may result in termination of services.
9. Scope of Practice Disclaimer
Mental health services do not guarantee outcomes. Diagnosis and treatment are based on professional judgment and patient-reported information.
House of Haven Mental Health does not provide:

Forensic evaluations

Court-ordered assessments

Disability determinations
-
Fitness-for-duty exams unless explicitly agreed upon in writing
10. Confidentiality & HIPAA
All patient information is protected under HIPAA and Kansas confidentiality laws. Information will not be released without written consent except as required by law (e.g., duty to warn, abuse reporting).
11. Website Use
